COVID-19 Test Results and Accessing your Medical Records

If you or someone in your household has been tested at HRRMC for COVID-19, do not come to the hospital to obtain your results. You will be contacted with your results. The department in which you were tested will contact you with the results (whether that is the Emergency Department or your provider’s office).

  • If you have access to your patient portal, lab results will be resulted there.
  • Patients needing copies of COVID-19 or other results can request paper copies by printing out the release of information form, filling it out, signing it, and emailing the completed form to the Health Information Management/Medical Records Department at records@hrrmc.net We will get your records to you.
  • Call us if you have any questions. The Health Information Management/Medical Records Department can be reached at (719) 530-2285, Monday – Friday, between 8:00 a.m. and 4:30 p.m.

How to obtain a copy of your clinic and hospital records at Heart of the Rockies Regional Medical Center

A patient or their legal representative may inspect and/or obtain a copy of their health information, or have copies of their records sent to another facility or provider. A completed and signed authorization form along with valid photo identification is required for copies of records to be released.

Here you'll find the "Authorization for Disclosure of Protected Health Information (PHI)" form. The form must be completed in its entirety. Incomplete forms will be returned, which may cause a delay in responding to your request.

Completed and signed “Authorization for Disclosure of Protected Health Information (PHI)” forms may be faxed, mailed, emailed or brought into the hospital’s Medical Records Department located on the second floor of the hospital near the elevators.

Valid identification

Medical records will only be released when the requester provides proper identification. Photo identification is required. If you are requesting records for someone other than yourself, the patient must have completed the authorization form giving you authority to obtain copies. In certain circumstances, you may be asked to provide supporting legal documentation such as:

  • Power of Attorney
  • Advance Health Care Directive
  • Living Will
  • Executor of the Estate
  • Court order/Conservatorship appointed by the court

How to obtain birth and death certificates

Heart of the Rockies Regional Medical Center does not retain copies of birth or death certificates. These records are maintained by Chaffee County Public Health.
